Sac Family Connect operates under an MOU comprised of over 30 agencies to help Sacramento County create a sustainable, unified system that supports families with the home visiting and family support services they need while maximizing available funding to serve more families.
All family support service providers in Sacramento County are encouraged to utilize Connecting Families with Services to make referrals. Family support programs are strongly encouraged to join as they are an essential part of the network of care.
First 5 Sacramento serves as the backbone organization and will work with partners to promote increased coordination while creating significant systems changes.
